My child has extracurricular activities during before or after school care, what do I need to fill in?

You must let the service know and complete the Extra-Curricular Release and Return Form.

If your child will attend our program, and then leave to attend activity during the session, the coach/ parent/ authorised person will need be available to sign the child out for the activity.  They will also need to be listed as a contact on your account and be authorised to collect the child from the program.

If the child is returning to TeamKids following the activity, they would need to make their way independently back to the program or be assisted by the coach or other authorised person.

Our team cannot assist with this process as we need to remain in ratio and leaving the space will not support this regulation.
